Michele Diana, MD, PhD, MBA presents his BJS Lecture "Image-enhanced and guided surgery" delivered at the Swiss Society of Surgery's Annual Meeting (held on May 29-31, 2024). Michele Diana is a surgeon and a translational scientist in new surgical technologies with expertise in medical device design and development. His main translational research interests include image-guided surgery, surgical robotics, and surgical applications of machine and deep learning (DL). During the past 10 years, together with a multidisciplinary team of engineers in biotechnology and computer science, he managed to attract several public and private grants to run innovative R\&D projects, leading to a substantial academic output (more than 250 publications in peer-reviewed journals), several awards, and some successful tech transfer cases. During the same period, he was invited to deliver more than 150 talks to international conferences, mostly as keynote or invited speaker. After a transformational Executive MBA experience at the INSEAD Business School (Fontainebleau, France), his current ambition is to move towards a more entrepreneurial career and create a new Venture Building structure with the objective to help closing the gap between academia and market.

In this educational lecture, Patricia Tejedor, Consultant Colorectal and Robotic Surgeon at University Hospital Gregorio Marañón in Madrid, Spain, explains propensity score analysis. This is a major statistical technique used in observational studies to reduce the risk of bias. It estimates the effect of a treatment by accounting for covariants which predict receiving the treatment and is a statistical technique with which surgeons should be familiar.

In this educational lecture, Mr. Martyn Evans, MB, MPhil, FRCS(Gen), consultant colorectal surgeon, and BJS Editor takes an in-depth look at randomized controlled trials (RCTs). He explains that planning the RCT is the fundamental stage to have any hope of achieving the answer to the scientific question posed. He gives wise advice on the planning, conduct, and writing up of the RCT that will increase the chances of successful publication in a high-impact journal.

As a consultant colorectal surgeon, Martyn Evans has an interest in advanced and recurrent pelvic malignancy, and is also Honorary Associate Professor at Morriston Hospital and Swansea University Medical School, a position he took up in 2021. Following medical school at Southampton University, Martyn trained in England and Wales, completing fellowships with the Swansea Pelvic Oncology Group and the John Goligher Colorectal Unit, Leeds. He has published over 100 peer-reviewed papers and has been a BJS Editor since 2018.

Surgeon scientists need to understand some basic research, but also when to request help from a statistician. Statisticians always prefer to be involved with the design and planning of the project, not just being involved at the stage of the analysis.&#x20;

Jonathan Cook is a Professor of Clinical Trials and Medical Statistics. His main research interest is in the design, conduct, analysis and reporting of randomised controlled trials. He graduated from the University of Aberdeen with a BSc in Statistics . Subsequently, he worked at the Health Services Research Unit, University of Aberdeen. Jonathan joined the Centre for Statistics in Medicine, University of Oxford as a University Lectureship in 2013 . He has collaborated on numerous projects such as randomised trials, observational and diagnostic studies, methodological projects and systematic reviews in a variety of clinical areas (including general surgery). Jonathan has over 300 publications including more than 60 as first or senior author, and he recently authored an introductory book on clinical trials published by Oxford University Press (OUP).

In this educational lecture, Professor Ville Salinnen, MD, PhD, Editor-in-chief of BJS open, thoroughly describes how to conduct surgical research and get it published in a respectable journal. This Acta lecture was presented at the Swedish Surgical Week in 2024. Professor Salinnen notes that successful publication starts with a good study plan, completion of the study and then writing the paper. He offers a detailed explanation about how to construct and submit the paper for publication.

This lecture by Dr. Teresa LaMasters from Iowa, USA, past President of ASNVS, discusses surgical burnout. It was originally given as the BJS lecture for the International Bariatric Club in 2023, but is still very relevant. Burnout is a problem among surgeons, and importantly, can affect patient safety. Dr. LaMasters looks at the causes, those most at risk, how to recognize burnout, and how to help an affected individual. She reminds us that surgeons need to work together.

This authoritative BJS lecture was delivered by Professor Eduardo Targarona, MD, PhD, FACS from the Hospital Sant Pau in Barcelona (Spain) at the European Hernia Society in 2023. Professor Targarona discusses the history of evidence-based surgery and how that has directed a move from open surgery to a more minimally invasive approach. He discusses the rise of robotic surgery and the digital revolution in computing power that enabled this. He outlines how training in minimally invasive surgery can be obtained, and what good training looks like. Finally, he looks into the future about how artificial intelligence (AI) could help guide surgical procedures and optimize some of the academic aspects of surgical research and surgical progress.

What does it mean to be an (academic) endoscopic surgeon? 

This keynote lecture was delivered by Stephen Wigmore, KHS, FRCSEd, FRCPE, FRSE at the Surgical Research Society’s Annual Meeting held at St Catharine’s College in Cambridge (United Kingdom) from January 11 to 12, 2024.&#x20;

The development of novel cell- and tissue-derived therapies has placed a particular responsibility on the surgical community to understand the regulations, which have been designed to apply to such treatment approaches. Surgeons may be involved in almost every step of the pathway to develop and use cell- and tissue-derived treatments. Regulations have been designed to protect patients and cover the licensing, safety, and use of novel technologies. In this key lecture, I will present real-world examples of the use of advanced therapy medicinal products and other cell-based surgically applied therapies and show how mistakes can be made leading to unsafe products and patient harm. Regulation is not just limited to cell-based therapies, but also extends to mathematical or artificial intelligence driven algorithms, which are used to assist clinical decision-making. Again, an example of an algorithm in clinical use, which has suffered criticism for inherent bias has been given.

The confusion that exists in regulatory pathways is highlighted, and the explosion of products for surgical placement using tissue scaffolds, autologous or allogeneic and even manufactured cells demands a rethink of how regulatory approval is achieved for new products and treatments. A case is made for linking research and regulatory milestones so that regulatory approval becomes an intrinsic component of product development rather than an afterthought once development has already occurred. Staying on the right side of the law and protecting patients are critical requirements for all surgeons, and thought needs to be given about education in this rapidly developing area.

Future surgical innovation and the importance of regulation

In this keynote lecture, Julie Hallet and Susan Hallbeck explore the role of intraoperative microbreaks in improving operating room ergonomics, reducing OR staff discomfort and enhancing mental focus. They review research and evidence demonstrating how these breaks can improve surgical accuracy and performance and, ultimately, benefit surgeon wellness and patient safety. They also describe practical applications to facilitate these microbreaks in the OR , including a web-based app designed specifically for this purpose.
